Community Connections

At WellCare of Kentucky, we understand much of what impacts health happens outside of the doctor’s office.

That’s why we go beyond healthcare. We help our members manage the many challenges of daily life.

Community Connections offers two avenues of member connectivity:

  • Call our Community Connections Help Line, or
  • Connect directly with the community based organizations that our Community Connections Field Teams partner with

Community Connection Help Line

Peer Coaches provide a comprehensive needs assessment. They then refer members to organizations that can address their needs. During the call, Peer Coaches can check for and assist in closing care gaps. Within a couple weeks, they follow up to make sure that the member’s needs were met.

We look to address the social determinants of health (SDOH) for our members. We do this by studying of the effectiveness of the services we provide to our members. We also assess healthcare costs, utilization and other health outcomes.

Our Community Connections Model connects our members with resources. We understand the needs of our members.  These resources assist our members with services, such as:

  • Housing
  • Transportation
  • Food assistance

This helps pave a path toward better health and independence. These resources can be accessed through our community partnerships. Members can also call our Community Connections Help Line.

Community Connections Field Team

We collaborate with community based organizations (CBOs) around core issues of concern, including:

  • PCP connectivity
  • Workforce innovation
  • Housing insecurity

This allows us to track member services and their effect on outcomes.

Help with Problems Beyond Medical Care

It can be hard to focus on your health. It can be harder if you have problems with your housing or worry about having enough food to feed your family. WellCare of Kentucky can connect you to resources in your community. We can help you manage issues beyond your medical care.

Call our Community Connections Help Line at 1-866-775-2192 to talk to a Peer Coach if you:

  • Worry about your housing or living conditions
  • Have trouble getting enough food to feed you or your family
  • Find it hard to get to appointments, work or school because of transportation issues
  • Feel unsafe or are experiencing domestic violence (if you are in immediate danger, call 911)
  • Have another type of need such as:
    • Financial Assistance (utilities, rent)
    • Affordable childcare
    • Job/education assistance
    • Family Supplies – diapers, formula, cribs, and more
