As a member of WellCare of Kentucky, you/your child have dental benefits.
To make it easy to get the needed care, we are pairing you/your child with a dentist that will be your Dental Home.
A Dental Home is the dental office you/your child get dental care at. If you/your child need a check-up or have tooth pain, contact your Dental Home, and they will take care of you/your child.
Getting regular dental care is important because it can prevent painful dental issues.

We will send you a notification soon with the name, address, and phone number of your Dental Home.
If you/your child has already had care with a dentist, you/your child will be paired with the same dentist. All family members will be kept together. As always, you may choose to see any in-network provider of your choosing. If you would like to change your dental home or need help finding a new dental provider, you may do so at any time by calling Avesis Member Service at 1-855-704-0432..
